This is the fifth webinar in our Medication safety educational series. The session will cover:

  • characterising anti-rejection-related medication incidents into themes and subthemes 
  • describing factors that contribute to anti-rejection medication-related errors 
  • highlighting strategies and practices for preventing common errors associated with anti-rejection medications. 


The Institute for Safe Medication Practices Canada will present on Errors in the management of anti-rejection drugs in patients who have undergone organ transplant, which can lead to loss of the grafted organ or increased susceptibility to infections. This session will describe an analysis of anti-rejection medication-related errors, both quantitatively and qualitatively, with a focus on medication incident examples, potential contributing factors, and practical strategies to prevent recurrence.
