Description

Spread over two half days, the training will go through the root cause analysis (RCA) methodology for health service staff and consumer advocates in reviewing serious and sentinel events.

NOTE: In order to attend this workshop, evidence must be provided of attendance at the Fundamentals of Adverse Patient Safety Event Reviews Workshop (or Incident Review workshop) within the last 3 years and have received a certificate of attendance.

Who should register?

Health service workers from in scope Victorian health services, and consumer representatives who may participate in reviewing a sentinel or serious adverse event in Victoria.

Please note: In scope Victorian health services are identified in the SCV Policy: Adverse patient safety events policy (2019) as follows:

  • Ambulance Victoria
  • Bush nursing centres
  • Forensicare (Thomas Embling Hospital)
  • Public sector residential aged care facilities
  • Hospital in home services
  • Private day surgery facilities

Note: As capacity is limited, participants are selected and prioritised according to need. We will email you the outcome of this EOI process.

Confirmation of attendance: Successful Participants allocated a place will receive an email with pre-reading materials and other information prior to the workshop.
