Healthcare is multifaceted, fragmented, busy and expensive. As we age, our health needs become more complex. Older people suffer a disproportionate amount of harm in our health system, often unrelated to their illnesses.
The Institute for Healthcare Improvement (IHI) has developed an Age-Friendly Health System '4Ms Framework' for the care of older people. This evidence-based set of integrated, core elements reduces hospital harm and costs, and improves health outcomes.
This webinar will provide an overview of the Building an Indigo Age-Friendly Health system project.
The project, funded through the Better Care Victoria Innovation fund, examines the suitability of the IHI approach for implementation into the Victorian rural health setting.
