Safer Care Victoria is looking for health services to participate in a pilot project for a Falls review tool to help improve outcomes after falls related adverse events.

In 2019-20, there were 25 sentinel events reported to SCV that were directly attributed to falls. We are committed to improving outcomes from falls adverse event reviews by applying a systems-focused approach and strengthened review capability of health services. The Falls review tool aims to assist those who review falls by using human factors and systems thinking to improve the quality of review and development of recommendations as the result of these events.

Join our expert working group

We're seeking representatives from a range of Victorian Health Services (public and private, large and small, metropolitan, rural and regional) to join our working group to help pilot the Falls review tool. 

As a member of the working group, you will be able to use your experience as clinical governance and patient safety experts to test the tool within your health service, and to provide valuable feedback.  You will also engage a consumer to understand how consumers may have been affected by a fall. 

The pilot will run between April 2022 and July 2022. 

Who should apply?

All Victorian public and private health services are eligible to apply. We are looking for a range of health service sizes and locations. 

Expressions of interest will be received until 5.00 pm 8 April 2022

How will sites be selected? 

Site selection will be based on achieving diverse representation from Victorian health services. The decision will be at the discretion of SCV.

What’s involved?

Interested services are asked to nominate a staff member with expertise and current (or very recent) experience leading reviews of adverse patient safety incidents. Nominated staff members will attend 3 to 5 online working group meetings run by SCV, along with staff from other participating services.

Participating services will pilot the review tool at their respective sites from April to July 2022 and help identify refinements and improvements. 

Services will use the tool to review at least three (3) of their historical events and any relevant events that occur within the pilot period. Health services will also table the tool at their respective Falls committee and speak to a healthcare consumer about their experience of falling and falls prevention within their health service.
