Cardiovascular disease is a significant burden on the lives of Victorians, with many experiencing multiple hospital admissions and challenges in accessing specialist cardiac care in hospital and community care settings.
We are partnering with 30 Victorian health services and the Institute for Healthcare Improvement to embed evidence-based models of care that improve the management of, and increase the time spent in the community for, those living with heart failure.
We will also offer dedicated funding for the establishment of cardiovascular liaison nurse positions to further drive cardiovascular improvement initiatives within health services.
This statewide initiative will run for 18 to 24 months and is part of the Reducing avoidable acute care admissions package along with the Heart helper pilot.
