On 11 February 2026, representatives from 25 Victorian health services participating in the Preventing Venous Thromboembolism and Supporting Anticoagulation Management (PVT‑SAM) Collaborative came together for their second learning session, with more than 90 clinicians and consumers in attendance.
The session brought participants together around the Collaborative’s two key aims:
- reducing venous thromboembolism (VTE) in perioperative patients
- minimising inappropriate anticoagulant prescribing in medical patients.
Health service teams shared updates on their progress to date, highlighting early achievements and insights emerging across sites. Storyboards and spotlight presentations demonstrated a range of tested change ideas, local challenges, and examples of improvement in VTE prevention already taking shape.

A strong focus of the day was on building capability in using data for real‑time improvement. Participants explored how measurement can guide decision‑making, track progress and support teams to refine and scale change throughout Action Period 2.
The program also delved into the human side of change, with discussions and activities centred on understanding behaviours, supporting engagement in change ideas and strengthening collaborative practice across multidisciplinary teams.
The session concluded with team‑based action planning, enabling services to identify change priorities, consolidate learning and prepare for the next phase of implementation.

Teams will continue their work through Action Period 2, focusing on refining tested change approaches, scaling effective changes and driving further improvements in VTE prevention and anticoagulation safety across Victoria.
