In the last 12 months, the Safer Together Program (STP) has achieved strong early results across all four of its strategic priorities.
Through the collective efforts of project teams across Victoria, the program has:
- Positively impacted the care of more than 5,500 patients
- Saved 2,143 bed days through better models of care and improved patient preparedness
- Avoided significant costs across the system
- Actively partnered with more than 20 consumers
- Established connections with all 76 public health services in Victoria.
Spotlight on Health Service Partnership STP Teams
At the heart of these outcomes has been the Health Service Partnership STP Teams — the foundation of our connected delivery model and driving force behind the emergence of a collaborative learning health system in Victoria.
On Tuesday 17 June, we hosted a gathering of all 8 teams to mark the program’s one-year milestone. The event was a powerful reminder of what’s possible when teams come together with a shared commitment to safer, more connected care.
Throughout the day, we heard inspiring stories of local impact, shared learning, and authentic collaboration that has taken root across the state.
Over the past year, these teams have supported the program by building strong relationships, enabling peer learning, and tailoring support to meet the unique needs of each region.

What’s next
In the year ahead, the program will be delivered across all 12 Local Health Service Networks (LHSNs).
There will be new opportunities to get involved in your region — whether through LHSN STP team recruitment opportunities, STP priority project participation, capability building opportunities, peer learning sessions, or collaborative events.
