Developed with Victorian consumers and health services, the toolkit provides practical strategies, tools and real-world examples to help acute health services strengthen asthma care, improve care transitions and support children and families to manage asthma effectively.
The toolkit also includes resources that may be useful for primary and community healthcare providers.
