We recently delivered the third and final learning session for the Preventing Venous Thromboembolism and Supporting Anticoagulation Management (PVT-SAM) Collaborative, marking the conclusion of a journey of learning, improvement and collaboration across Victoria.

More than 70 clinicians, consumers and improvement leaders from 23 Victorian health services came together to share insights from their improvement journey, share lessons learned and work together to sustain improvements beyond the life of the Collaborative.

Over the course of the Collaborative, teams worked to:

  • reduce venous thromboembolism (VTE) in perioperative patients
  • improve appropriate anticoagulant prescribing in medical patients.

Thank you to all participants for your commitment, expertise and willingness to learn from one another. 

Together, you have strengthened VTE prevention and anticoagulation safety across Victoria, creating a foundation for safer care that will continue well beyond the Collaborative.
