Safer Together Program

More than 100 participants from 25 Victorian health services recently came together with a shared goal of reducing venous thromboembolism (VTE) in perioperative patients and minimising inappropriate anticoagulant prescribing in medical patients.

Our first learning session for the Preventing Venous Thromboembolism and Supporting Anticoagulation Management (PVT-SAM) collaborative saw teams sharing insights, experiences and collaborating with such energy and enthusiasm.

Since then, participating health services have begun testing change ideas, gathering data, and continuing to learn from each other – all with the goal of driving meaningful and lasting improvements.
