    To highlight the limitations of mobile devices for communicating clinical concerns unless a direct phone call is made.

    Background

    Mobile phones set to silent mode or low volume, text messages not received or detected and delay in receipt of voice messages have all been associated with delays in patient treatment. Sometimes, this can lead to devastating consequences. Effective communication between junior and senior medical staff, as well as between nursing and medical staff, is essential for care planning and timely notification of patient deterioration. This information exchange is predominantly conducted via (mobile) phone. The Surgical Subcommittee has identified a case where text messaging failed to convey the evolving clinical scenario adequately, potentially contributing to a delayed recognition of complications. 

    Recommendations

    The VPCC recommends that:

    • A message, either text or voicemail, should not be considered received until confirmation is provided by the intended clinician.
    • Voice calls rather than text are strongly preferred in time-critical situations
    • Clinicians anticipating limited mobile access should organise alternative communication pathways or delegate patient care responsibilities to a colleague for the duration of theirunavailability,

    1. Verify device status

    Ensure the phone is powered on and sufficiently charged. Check for active modes such as Do Not DisturbSilent, or Airplane Mode.

    • iOS: Swipe down from the top-right to check Control Center.
    • Android: Swipe down from the top to view Quick Settings.

    2. Review notification settings

    Confirm that Do Not Disturb is disabled and that calls and alerts are allowed.

    • iOS: Settings → Focus → Do Not Disturb.
    • Android: Settings → Notifications → Do Not Disturb.

    3. Confirm app permissions

    Ensure clinical apps (e.g., secure messaging, paging) have notification and background access.

    • iOS: Settings → [App Name] → Notifications.
    • Android: Settings → Apps → [App Name] → Notifications.

    4. Check Automation and Schedules

    Disable or adjust any automation features that may suppress alerts during on-call hours.

    • iOS: Settings → Focus → Automation.
    • Android: Settings → Rules or Schedules (varies by manufacturer).

    5. Pause automatic updates

    To prevent unexpected restarts, pause or schedule updates outside on-call hours.

    • iOS: Settings → General → Software Update → Automatic Updates.
    • Android: Settings → System → Software Updates → Auto-download over Wi-Fi.
