Rapid Access Atrial Fibrillation Clinics Project

COMPLETED

Our goals

To improve the rates of patients who receive evidence-based care for atrial fibrillation and are prescribed appropriate anticoagulation.

About the project

Atrial fibrillation (irregular and often rapid heart rate) is the leading cause of stroke and a frequent cause of hospital admission for Victorians. Access to timely and focused healthcare following a heart event can dramatically reduce the risk of stroke and poor health outcomes.

Guiding the safe use of bed rails

Complete

In partnership with clinicians, we identified variation in the use of of bed rails across Victorian healthcare services and public residential aged care facilities.

In Australia and overseas, inappropriate  use of bed rails has been known to cause injuries and deaths when people have attempted to climb over or through gaps in the bed rails.

Telemental health pilot (Tele-HELP)

COMPLETE

This was an innovation project supported by the former Better Care Victoria Innovation Fund.

Lead organisation

Ambulance Victoria

Summary

Many of the mental health patients who call Triple Zero (000) for assistance would receive more appropriate care from a specialist mental health clinician than from the paramedics that staff a typical ambulance.
