Hosted by Loddon Mallee Local Health Service Network, this Clinical Conversation will explore a new role developed in response to the growing numbers of older patients experiencing cognitive and functional decline, carer stress, and behaviours of concern. The need for alternative models of care in the acute setting has led to the introduction of a dedicated staff member focused on patient engagement and wellbeing.
The trial has demonstrated significant outcomes: reduced hospital-acquired frailty, improved patient experience and health outcomes, decreased nursing workload, and a more positive and inclusive ward culture. Presenters from Bendigo Health will share how these results were achieved through meaningful group activities and individualised 1:1 support, with the role of the Lifestyle Coordinator introduced later in the session.
