We are pleased to spotlight Eastern Health’s local interdisciplinary innovations as part of the Safer Together Program. Join our presenters as they share practical insights from two initiatives:
- Interdisciplinary GEDI model: Specialists in older person embedded in Emergency Departments deliver early, expert assessments to divert patients from unnecessary acute admissions to more appropriate care pathways or home-based services. This model has diverted 70% of older patients reviewed from acute admission, saving over 5,000 bed days and nearly $3.5 million in costs.
- Red to Green bed day process in subacute care: A daily visual management system that identifies and addresses barriers to patient care progression, supporting collaborative timely decision-making and reducing subacute inpatient length of stay by nearly five days.
