Hosted by South Metro Local Health Service Network, this Clinical Conversation explores a renewed, data‑driven and sustainable approach to improvement following Serious Adverse Patient Safety Events (SAPSEs) at Monash Health.
The Monash Health Quality & Clinical Governance team will introduce their pilot model, which shifts away from generating large volumes of individual recommendations and instead focuses on addressing recurring system‑level themes through targeted SAPSE Working Groups. Presenters will step through why this change was needed, how clearer governance, priority theming and structured processes were established, and how this approach is strengthening system learning, reducing duplication of effort and preventing recurrence of harm.
