In this Clinical Conversation, South West Healthcare will share the story of a 12-month project focused on strengthening VTE prevention and anticoagulation management across their organisation. As part of a broader statewide improvement initiative, the team had opportunities to learn alongside other Victorian health services, while adapting those learnings to meet the needs of their local regional context.
Using the project as a case study, presenters will reflect on the importance of multidisciplinary relationships, clinical engagement and ongoing learning when implementing change. They will discuss what was trialled, what they learned along the way, and how the project has continued to influence VTE prevention and anticoagulation management at their health service.
