Description

This clinical conversation will showcase an innovative program partnering with rural communities to support farmers to access health care in a setting they already trust and frequent, their local livestock saleyards.

Register now for this free webinar and secure your spot to hear firsthand about this groundbreaking approach to rural healthcare.

By engaging farmers during routine cattle sales, the program aims to improve both physical and mental health outcomes and reduce avoidable hospital admissions. We are pleased to spotlight Yea & District Memorial Hospital’s Yea Saleyards Farmer Healthcare Program, the first of its kind in Australia, delivered in partnership with the Murrindindi Shire Council. Join our presenters as they share YDMH’s journey in shifting the delivery of care from traditional health service settings to the local saleyards, where farmers naturally gather, connect and socialise. This program demonstrates how a relational, place-based model of care, built through collaboration between farmers, clinicians and local partners, can significantly improve access to both physical and mental healthcare and health outcomes in rural communities. Presenters will also introduce the Implementation Guidelines that have been developed to support other health services who may be interested in replicating this highly successful model in their rural or regional communities.

This webinar is part of the Safer Together Clinical Conversations series, where we partner with Local Health Service Networks to showcase local improvement work and share practical learnings that others can adapt in their own settings. Together, these conversations help connect health services across Victoria through a continuously improving learning health system.
