Hosted by the Grampians Local Health Service Network as part of the Safer Together Clinical Conversations series, this session will showcase a first‑of‑its‑kind Regional Stroke Coordinator role, introduced to better connect services, strengthen stroke pathways, and support faster access to lifesaving treatment in regional communities.
Stroke care is highly time‑critical, and variation in access can significantly impact patient outcomes. In response, the Grampians region piloted a coordinated, system‑wide approach to improve how services work together, supporting more timely, consistent and equitable care.
This session will explore the implementation, impact and key learnings from the 12‑month pilot, including how the Regional Stroke Coordinator role is helping to drive improved coordination, earlier recognition, and faster treatment for patients across the region.
Presenters will also share how a combination of system‑level changes and community awareness approaches have contributed to improving recognition of stroke and strengthening access to care.
