Hosted by Parkville LHSN as part of the Safer Together Clinical Conversations series, this session will begin with a presentation from the Royal Children’s Hospital (RCH) Melbourne, showcasing strategies implemented to reduce the risk of intravenous (IV) medication errors.
The RCH team will then be joined by pharmacists from the Royal Melbourne Hospital and the Royal Victorian Eye and Ear Hospital for a panel discussion. Together, they will discuss how medication safety strategies are developed in response to safety signals, and how different services are applying and adapting approaches to suit their own patient populations and care settings.
Through sharing their experiences, the panel will highlight practical insights that can be applied across a range of health service contexts. This content and the learnings shared can be applied at any health service and will be relevant to pharmacy, nursing, and quality and safety staff and leaders.
