ViCTOR Chart Rollout - From Pilot to Practice: A Sustainable Implementation Approach Shaped by the Hume Local Health Service Network (LHSN)

This Clinical Conversation showcases Hume LHSN’s leadership in coordinating the rollout of the updated Victorian Children’s Tool for Observation and Response (ViCTOR) chart across all health services in the region. 

This session will:

• demonstrate the practical, evidence based model used to support Hume LHSN health services through implementation
• showcase how a network‑led approach can enable effective scale and spread across diverse services
• highlight the new consumer escalation question and how tailored education and tools are helping clinicians embed these updates into daily practice.
