We have developed a practical guide to support the statewide implementation of the refined Victorian Children's Tool for Observation and Response (ViCTOR) chart.

The updated chart introduces a new vital sign, “Are you worried your child is getting worse?” reflecting emerging evidence that a worried parent can be as effective as standard vital signs in identifying children at risk of deterioration.

The resources provide practical guidance on implementation, including:

  • identifying site champions
  • timing for asking the question
  • escalation protocols
  • training
  • electronic medical record (EMR) system integration.

They aim to support consistent adoption across Victorian health services and strengthen the safety and quality of paediatric care.
