Overview
The Safer Together Program is our statewide safety improvement program. It aims to establish a continuously improving learning health system that is safer, more person-centred, sustainable, and delivers better outcomes for Victorians.
We will do this by building and leveraging our capability to know and enable what matters, and support you to deliver what matters, in partnership with consumers, clinicians, health services, system leaders and researchers.
Through the program, we work with the health sector and consumers to:
- design, implement, and evaluate quality improvement projects
- share quality improvement findings, insights, and resources
- support a learning health system.
Our focus
- Supporting the implementation of consistent, evidence-based practice
- Strengthening integrated and preventative care pathways
- Ensuring safe medication practices
- Focusing on necessary & effective interventions
Priority outcomes
The program's strategic priority outcomes include:
Our approach
The program uses a networked approach to balance the need for statewide consistency and local adaptation. This helps us to:
- improve communication with health services.
- reflect regional perspectives in program design and delivery.
- accelerate learning by identifying local successes and sharing them across the system.
- achieve equitable health outcomes for Victorians.
Who we work with
To achieve effective and lasting improvements, we collaborate with:
- Healthcare services
- Consumers
- Clinician Leaders
- Local Health Service Networks
Our improvement approach
We use improvement science and the Model for Improvement.
For each improvement project, we partner with the sector and consumers to:
- Understand systemic issues
- Design an improvement approach and measurement strategy informed by clinicians and consumers
- Plan and implement changes within participating health services
- Measure the effectiveness of these changes
- Share the outcomes, insights, and lessons learned
- Produce toolkits for other healthcare services to undertake improvement work in their own service
