100,000 Lives – Impact report 2023–24 released
We’re proud to share our 2023–24 impact report for the 100,000 Lives program. This year, through a range of small and large-scale improvement and innovation projects, we have enhanced the lives of 18,456 Victorians. The report showcases key achievements, highlighting our collaborations with health services, consumers, and experts to drive meaningful change.
Future Health Today
About the project
The Future Health Today project aims to enhance chronic disease management through the Future Health Today (FHT) implementation program – a new clinical software tool, coupled with integrated tertiary health service support, to assist general practitioners to screen, diagnose and manage heart disease, diabetes and kidney disease before complications occur.
