Safety Culture in Birth Suite

About the project

In this pilot project, we are partnering with 5 Victorian maternity services to strengthen safety culture in birth suites. The project focuses on practical ways to help maternity teams work together, communicate openly, and respond quickly to risks – making care safer for mothers and babies.

Chronic Obstructive Pulmonary Disease (COPD) Acute Care Collaborative

The Chronic Obstructive Pulmonary Disease (COPD) acute care collaborative aims to improve the experience and outcomes for people with COPD in Victoria.  It will address the critical factors that increase the likelihood of readmission to hospital from COPD.

We are partnering and providing funding to Lung Foundation Australia to deliver a nurse-led collaborative aimed at improving patient experience, quality of care and health outcomes for people with COPD in Victoria.

Assisted Self-Management

About the project

The Assisted Self-Management pilot is a system-wide improvement initiative designed to support people with chronic conditions to stay well at home and avoid unnecessary hospital admissions. 

This pilot will test a digitally enabled, patient-centred care model that combines clinician oversight and support with evidence-based self-management strategies.
